بررسی رادیوگرافی اثر پلاسمای غنی از پلاکت بر ترمیم استخوان در محلّ برداشت سر استخوان ران در خرگوش

چکیده

پلاسمای غنی از پلاکت (PRP) به‌عنوان یک مکمل امیدبخش در درمان‌های ترمیمی اسکلتی-عضلانی مطرح شده است. جراحی برداشت سر استخوان ران (FHO) تماس استخوانی بین سر استخوان ران و حفره استابولوم را محدود کرده و امکان تشکیل یک شبه‌آرتروز فیبری را فراهم می‌کند.
این مطالعه با هدف ارزیابی اثرات رادیوگرافی PRP بر ترمیم استخوان پس از استئوتومی سر استخوان ران (FHO) در خرگوش‌ها انجام شد. هجده خرگوش نر بالغ سفید نیوزیلندی به‌طور تصادفی به دو گروه مساوی درمان و کنترل تقسیم شدند. شش سی‌سی خون از قلب خرگوش‌های گروه درمان در لوله حاوی ماده ضدانعقاد جمع‌آوری شد و پس از دو مرحله سانتریفیوژ و فعال‌سازی با CaCl₂،  PRP اتولوگ تهیه گردید. برای هر خرگوش در گروه درمان، 1.5 میلی‌لیتر PRP در محل جراحی استفاده شد.
ارزیابی‌های رادیوگرافی در هفته‌های دوم و سوم پس از عمل برای بررسی تشکیل کالوس، واکنش پریوست و تغییرات حفره استابولوم انجام شد. نتایج نشان داد گروه تحت درمان با PRP در مقایسه با گروه کنترل، بازسازی استخوانی بهتری شامل شروع سریع‌تر تشکیل کالوس و بهبود واکنش پریوست داشت. همچنین تغییرات حفره استابولوم در این گروه کمتر بود.
این یافته‌ها نشان می‌دهد PRP بهبود اولیه استخوان را تسریع کرده و یکپارچگی ساختاری را پس از FHO بهبود می‌بخشد و می‌تواند از کاربرد آن در جراحی‌های ارتوپدی حمایت کند. وجود غلظت بالای فاکتورهای رشد در PRP، علاوه بر تسریع ترمیم استخوان و تشکیل مفصل کاذب از طریق کاهش التهاب مفصل، می‌تواند در افزایش سریع‌تر تحمل وزن مؤثر باشد. برای تأیید نتایج، انجام مطالعات بیشتر بافت‌شناسی توصیه می‌شود.

Platelet-rich plasma (PRP), has emerged as a promising adjunct in musculoskeletal regenerative therapies. The FHO limits the bony contact between the femoral head and the acetabulum and allows the formation of a fibrous pseudoarthrosis. This study aimed to evaluate the radiographic effects of PRP on bone repair following femoral head osteotomy in rabbits. Eighteen adult male New Zealand White rabbits were randomly divided into two equal groups: a treatment and a control group. Blood was collected from the hearts of the treatment group in a tube containing anticoagulant in an amount of 6 cc, and after two centrifugation steps with CaCl2, the autologous PRP was activated. For each rabbit in the treatment group, 1.5 ml of PRP was prepared and added at the surgical site. Standardized radiographic assessments were performed at 2, and 3 weeks postoperatively to evaluate callus formation, area of periosteal reaction, and degree of changes in the acetabulum. The PRP-treated group demonstrated significantly enhanced radiographic signs of bone regeneration, including earlier onset of callus formation and improved periosteal reaction, degree of changes in the acetabulum compared to controls. These findings suggest that PRP accelerates early bone healing and improves structural integrity post-FHO, potentially supporting its use in orthopedic surgical practice. The presence of high concentrations of growth factors in platelet-rich plasma to accelerate bone repair and pseudo-joint formation by reducing joint inflammation, can be effective in faster weight gain in cases requiring hip arthroplasty. More histological studies are recommended to confirm these results.
